To donate your breast pump to a needy family, please fill out the form below, along with your full name and address. Then ship your breast pump to the address provided below. Once we receive the breast pump, we will make the arrangements necessary for your pump to go to a qualified family.

Also, if you know of a family in need of a breast pump that would meet the qualifications listed below, complete the following and we will begin the selection process.

Breast Pump recipient qualifications:

• Families with a child under the age of 2;
• Family with financial needs (proof required) or;
• The child must have been prematurely born or born with a chronic illness (verifiable by a physician, nurse, or social worker from a certified hospital, clinic, or health center).
